Please print and fill out the five (5) electronic forms below. This paperwork will take about 15-20 minutes to complete. If you are unable to download or print these documents, please contact us and we will send you copies in the mail. If you wish to fill them out at the office instead, please arrive 20 minutes early for your first visit.
Your initial visit may last up to two hours, and it will include an intake interview, evaluation, and initial treatment. Subsequent visits will last one hour, and they will include a brief follow-up evaluation and treatment.
Missed or cancelled appointments with less than 24 hour notice incur a $150 fee.
